The form can either be submitted electronically to Sunset Lakes Developments at sunsetlakes@rogers.com or a hard copy can be printed and submitted by delivery, mail or fax  to Sunset Lakes Developments, 1705 Old Prescott Road, Greely, Ontario, K4P 1M8 (fax no. 613-821-7997).  Please allow up to 30 days for a response.
